How to avail cashless service at network hospital?
  1. Policyholder approaches the network hospital with AITL ID card, treatment details along with prescriptions of referring doctor.
  2. For Planned hospitalization in a network hospital, the request for authorization letter (RAL) to be forwarded in specified format to AITL, 7 days prior to admission. The RAL should have all relevant details of complaints with duration, past relevant history, probable diagnosis. RAL should be completely filled clearly mentioning Yes or No in all columns, Contact details of treating consultant & Insured, duly signed. Along with Photo ID of insured/patient self attested also required (Apart from AITL card) duly verified by hospital.
  3. For Emergency hospitalization in a network hospital, the RAL to be forwarded in specified format to AITL within 24 Hrs. of hospitalization. The RAL should contain all details with contact no. of treating consultant & insured/patient duly signed along with photo ID of insured/patient (Apart from AITL) duly verified from hospital. In case of RTA/MLC, all relevant sections mentioned in the format should be filled with any history of Alcohol or inebriating drugs or any other addictive substance or resorting to intentional self injury to be clearly mentioned.
  4. AITL would issue Guarantee of payment letter only for medically necessary treatment, cost of implant if any would be mentioned in RAL to be paid by insurer. Non payable items as per policy terms & conditions would be paid by insured. RAL would be valid for 15 Days from the issue.
  5. If the cost of treatment exceeds the authorization limit, request for enhancement of authorization shall be made immediately during hospitalization using same RAL & providing valid reasons for enhancement of amount and duration of stay in hospital.
  6. AITL would evaluate the request again based on availability of further limits & issue the guarantee of letter again.
  7. At the time of discharge insured/patient would sign the standard Discharge summary and standard Final hospital bill. Insured has to pay for all non payable items, deduction made, along with copayment if any by AITL as per the policy terms & conditions. The discount if any must be mentioned in the final bill.
  8. Claim form duly filled & signed by Insured. Hospital is required to submit all original documents including Discharge summary, Hospital bill with bill break up investigation reports, including X-ray/CT/MRI films, pharmacy bill, RAL’s issued by AITL and other insured documents as mention above to AITL within 3 Days for scrutiny and payment by the insurers.
